2. Who We Are
Vital Care SA provides disability support and aged care services in South Australia. We are proudly associated with Bamboo Home Support, a nationally recognised managed home care and NDIS support organisation. Some of our Home Care Package and NDIS managed services are delivered in partnership with Bamboo Home Support, and certain client information may be shared with them for care coordination and compliance purposes.

3. What Personal Information We Collect
We may collect: full name, date of birth, contact details, NDIS/HCP referral codes and plan details, nationality and preferred language, medical and health information, mental health information (voluntary), cognitive assessments, emergency contact details, authorised representative information, Power of Attorney and Advanced Health Directive documents, pension and financial information, and signed service agreements.
4. How We Collect It
We collect information directly from you via our onboarding form, enquiry forms, phone calls, from your authorised representative or care coordinator, from My Aged Care or the NDIS portal, from your GP (with consent), and through Bamboo Home Support.
5. Why We Collect It
To assess eligibility, develop your care plan, deliver NDIS and HCP services, coordinate care, manage billing, comply with regulatory obligations under the NDIS Act and Aged Care Act, report to the NDIS Commission and Aged Care Quality and Safety Commission, manage complaints, and improve our services.
7. Storage and Security
We use secure cloud-based systems, encrypted digital records, and locked physical storage. We retain information as long as legally required and destroy it securely when no longer needed.

10. Children's Privacy
Where we collect information about a child under 18, we do so through their parent, guardian, or authorised representative with additional care.
